There are many controversies surrounding methods of fixing the distal tibiofibular syndesmosis. Because placement of an anteromedial-to-posterolateral screw is technically very difficult, to the authors’ knowledge, it has never been presented before. There are, however, rare occasions when it may be helpful. The simple technique described here is a useful supplement to all orthopedic surgeons’ armamentarium.
